Agency Disclosure Robbins Cruise Connections, LLC (Agency) issues these travel documents in the sole capacity as agent for the airline, tour company, cruise line or other supplier as indicated in the travel document, brochure, correspondence or other communication. As the agent of the supplier Robbins Cruise Connections, LLC does not assume the responsibility for the cancellation or substitution of service by the supplier, financial default of the supplier, refunds or unused services to be provided by the supplier, or any obligation, service or monies not the specific responsibility of Robbins Cruise Connections, LLC. Insurance The Agency highly recommends the purchase of travel vacation insurance to protect yourself from any unforeseen events. ________I wish to accept/ decline trip cancellation insurance (circle one). Please contact agent to obtain a quote for travel Insurance. Refunds, Changes and Cancellations The Agency has advised me of the supplier’s refund charge and cancellation fees and penalties if any. I have been advised of the $25 per person cancellation fee due and made payable to Robbins Cruise Connections, LLC if my reservation is cancelled any time prior to final payment (separate from cruise line cancellation penalties)._____ I have been advised that cancellation, change or refund penalties do apply to the travel associated with these documents and I fully understand the terms and conditions of such penalties. Passport, Visa and Security Requirements _____ I have been advised of any passport, visa and security requirements and have also researched any need for these documents and procedures for my travel event.
